We aimed to provide a comprehensive descriptive account of these concepts as survivorship following LT progressed. Self-reported instruments, part of the cross-sectional study design, were used to gauge sociodemographic data, clinical characteristics, and patient-reported measures related to coping, resilience, post-traumatic growth, anxiety, and depressive symptoms. Survivorship timeframes were characterized as early (one year or fewer), mid (one to five years inclusive), late (five to ten years inclusive), and advanced (greater than ten years). Adult patients gain broader access to liver transplantation (LT) procedures through the utilization of split liver grafts, particularly when grafts are shared between two adult patients. While split liver transplantation (SLT) may not necessarily increase the risk of biliary complications (BCs) relative to whole liver transplantation (WLT) in adult recipients, this remains an open question. A retrospective review of deceased donor liver transplantations at a single institution between January 2004 and June 2018, included 1441 adult patients. SLTs were administered to 73 patients. SLTs employ a variety of grafts, including 27 right trisegment grafts, 16 left lobes, and 30 right lobes. Employing propensity score matching, the analysis resulted in 97 WLTs and 60 SLTs being selected. Biliary leakage was observed significantly more often in SLTs (133% versus 0%; p < 0.0001), contrasting with the similar rates of biliary anastomotic stricture between SLTs and WLTs (117% versus 93%; p = 0.063). Graft and patient survival following SLTs were not statistically different from those following WLTs, yielding p-values of 0.42 and 0.57, respectively. A review of the entire SLT cohort revealed BCs in 15 patients (205%), comprising 11 patients (151%) with biliary leakage and 8 patients (110%) with biliary anastomotic stricture; 4 patients (55%) demonstrated both conditions. Recipients who developed BCs exhibited significantly lower survival rates compared to those without BCs (p < 0.001). Multivariate analysis indicated that split grafts lacking a common bile duct were associated with a heightened risk of BCs. Summarizing the findings, SLT exhibits a statistically significant increase in the risk of biliary leakage when compared to WLT. Fatal infection can stem from biliary leakage, underscoring the importance of proper management in SLT.
The recovery profile of acute kidney injury (AKI) in critically ill patients with cirrhosis and its influence on prognosis is presently unclear. We explored the relationship between AKI recovery patterns and mortality, targeting cirrhotic patients with AKI admitted to intensive care units and identifying associated factors of mortality.
Three-hundred twenty-two patients hospitalized in two tertiary care intensive care units with a diagnosis of cirrhosis coupled with acute kidney injury (AKI) between 2016 and 2018 were included in the analysis. In the consensus view of the Acute Disease Quality Initiative, AKI recovery is identified by the serum creatinine concentration falling below 0.3 mg/dL below the baseline level within seven days of the commencement of AKI. Recovery patterns were categorized, according to the Acute Disease Quality Initiative's consensus, into three distinct groups: 0-2 days, 3-7 days, and no recovery (AKI persisting beyond 7 days). A landmark analysis using competing risk models, with liver transplantation as the competing risk, was performed to compare 90-day mortality rates in various AKI recovery groups and identify independent factors associated with mortality using both univariable and multivariable methods.
AKI recovery was seen in 16% (N=50) of subjects during the 0-2 day period and in 27% (N=88) during the 3-7 day period; a significant 57% (N=184) did not recover. Patients who failed to recover demonstrated a substantially increased risk of death compared to those recovering within 0-2 days, as evidenced by an unadjusted sub-hazard ratio (sHR) of 355 (95% confidence interval [CI]: 194-649, p<0.0001). The likelihood of death remained comparable between the 3-7 day recovery group and the 0-2 day recovery group, with an unadjusted sHR of 171 (95% CI 091-320, p=0.009). Analysis of multiple variables revealed that AKI no-recovery (sub-HR 207; 95% CI 133-324; p=0001), severe alcohol-associated hepatitis (sub-HR 241; 95% CI 120-483; p=001), and ascites (sub-HR 160; 95% CI 105-244; p=003) were independently linked to higher mortality rates.
Cirrhosis coupled with acute kidney injury (AKI) frequently results in non-recovery in over half of critically ill patients, a factor linked to poorer survival outcomes. Strategies promoting healing from acute kidney injury (AKI) could improve outcomes and results in this population.

Data from a longitudinal cohort of patients across a multi-hospital, integrated US health system provided the basis for this quality improvement study, which incorporated an interrupted time series analysis. Beginning July 2016, surgeons were obligated to measure the frailty levels of all elective surgery patients via the Risk Analysis Index (RAI), motivating this procedure.
